The Behavioral Health Care Advocate connects Medicaid enrollees with community services to improve stability and avoid institutionalization. Responsibilities include managing aftercare plans, conducting field visits, and acting as a clinical liaison between health plans and providers.
Requires a Master's degree or PhD in a behavioral health field and an active, independent license in New York (LMHC, LMFT, LCSW, or LP). Candidates must have 2+ years of related experience, a valid driver's license, and reside in or near Buffalo, NY.
