Guide families through a structured three-phase treatment model to prevent foster care placement. Conduct assessments and coordinate multidisciplinary interventions to ensure family stability and safety.
Requires a BSW or a bachelor's degree in human services and knowledge of the child welfare system. Fluency in Spanish is considered a plus, and the candidate must be able to travel citywide.
Guide families through a structured, three-phase treatment model: Initial, Baseline, and Stabilization. Conduct comprehensive assessments and develop individualized service plans. Coordinate multidisciplinary interventions tailored to each family's unique needs. Build meaningful relationships with families over an average case duration of 12 months. Collaborate with internal and external providers to ensure safety, stability, and well-being.
