The Admission Nurse coordinates the admission of patients into hospice services by conducting consultations and educating families. They perform thorough patient assessments and collaborate with an interdisciplinary team to establish care plans.
Candidates must be a Registered Nurse licensed in Texas with at least one year of nursing experience and case management experience. Proficiency in Microsoft Office and a valid driver's license are required.
