The RN Case Manager serves as the clinical anchor, coordinating care and guiding interdisciplinary teams for hospice patients. Responsibilities include conducting comprehensive assessments, adjusting personalized care plans, and educating patients and caregivers.
Candidates must hold an active RN license and be a graduate of an accredited nursing school. One or more years of experience in hospice or home health is preferred, along with strong clinical judgment.
What You Bring Active RN license in the state of employment Graduate of an accredited school of Nursing. One (1) + year Hospice, home health or related organization experience preferred. Compassion for patients and families navigating end-of-life care Strong clinical judgment and the ability to work independently in the field Excellent communication skills across care teams and families
