The PMHNP serves as the clinical anchor for a behavioral health and dementia care program, providing psychiatric evaluations and medication management via telehealth. They act as the Provider of Record for the CMS GUIDE model, overseeing clinical care plans for complex adult and geriatric Medicare patients.
Requires an MSN or DNP in psychiatric mental health nursing, board certification (PMHNP-BC), and active Maryland APRN and DEA licensure. Candidates must have at least 5 years of post-certification experience, specifically in geriatric psychiatry and substance use disorder treatment.
Role Overview Avail Health is building a behavioral health and dementia care program supporting Medicare patients with serious mental illness, complex psychiatric conditions, substance use disorders, and dementia-related needs. The Psychiatric Mental Health Nurse Practitioner (PMHNP) is the clinical psychiatric anchor of that program — delivering comprehensive psychiatric evaluation, medication management, and dementia care via telehealth within a highly coordinated interdisciplinary team.
In addition to direct clinical care, the PMHNP serves as the GUIDE Provider of Record for Avail Health's participation in the CMS GUIDE (Guiding an Improved Dementia Experience) model, providing clinical oversight and serving as the authoritative clinical voice within the program's dementia care framework.
You'll work closely with the LCSW, Dementia Care Navigator, and Care Coordinator, supported by an ambient AI scribe and technology-enabled workflows designed to reduce administrative burden and support high-quality virtual care delivery.
This is a foundational clinical role in a newly launching program.
