The Opportunity

We’re looking for a caring, energetic, and dependable Community Health Worker (CHW) to join our team at Vayu Health. In this role, you’ll work directly with our members, especially those who need extra support, and help them stay healthy by supporting and/or connecting them to the care, services, and community resources they need.

You’ll build real relationships, listen closely, and help our members feel seen and supported. You’ll be a key part of the care team, helping our members set goals, solve problems, and get the support they need to stay well.

This is a hybrid role that involves meeting members in their communities, home, and clinics (such as Open Door) in person or virtually via phone and telehealth — while also collaborating daily with your team (diabetes educator and nutritionist, behavioral health) in a virtual setting to help engage and move our members forward together.

What you’ll do

• Support Patients & Build Relationships
• Be a friendly and trusted go-to person for your members.
• Help members set goals that are important to them and take steps to reach them.
• Check in regularly, offer support, and help them feel motivated.
• Listen to their needs and make sure they feel heard and respected.
• Be able to maintain a high level of confidentiality.

• Connect People to Resources
• Help members find and use services like housing, food, transportation, and jobs.
• Go with them to appointments or help them make important phone calls.
• Help them apply for benefits or programs they’re eligible for.

• Work With the Care Team
• Work closely with behavioral health clinicians, diabetes educators, nurses, doctors, and other staff to make sure the member’s care is smooth.
• Share helpful updates about how the member is doing.
• Support members after hospital stays or during transitions.

• Meet People Where They Are
• Meet members in places that work best for them — their home, clinic, or community
• Attend community events, visit churches or food banks, and build relationships with local groups.
• Help identify people in the community who may benefit from extra care.

• Encourage Healthy Habits
• Talk with members about healthy habits, movement, stress, and managing chronic conditions.
• Share easy tips and information to help them take care of themselves.

• Outreach & Enrollment
• Help find people in the clinic or community who could benefit from support.
• Share flyers or visit local spots to spread the word about our services.
• Help patients sign up and get started with the program.

Education and Licenses Required

• Qualifications:
• Required:
• Minimum high school diploma or GED
• Valid California driver’s license, proof of insurance, and reliable transportation (as this position requires travel between clinics, member home, in the community to food banks, etc)
• Community volunteer experience
• Ability to complete the health tests to be cleared by each
• FQHC (may include: lab testing, physical, access to health records, etc).
• CHW certification or obtain within 18 months of hire
• Live in Humboldt County
• Preferred:
• Healthcare Experience (i.e. community-based or social services)
• Bilingual in English and Spanish
• Lived medicaid and/or chronic condition experience (especially diabetes) in self or family friend
