Requires a high school diploma or equivalent and two years of experience working with adults with mental illness. A valid NJ Driver's License with fewer than six points is required.
We are seeking a compassionate, community-focused professional to support individuals experiencing homelessness and serious mental illness through advocacy, crisis intervention, housing support, and direct case management services. The ideal candidate is organized, empathetic, and passionate about helping consumers achieve stability, independence, and recovery.
Case Manager Position Type: Full Time; 40 Hours Weekly Benefits Eligible: Yes; Full Benefits Location: Office located in Bayville; Travel throughout Ocean County Department: Adult Residential & Outreach Services Salary: Identify housing needs and barriers to permanent housing while providing advocacy, referrals, and linkage to community resources to prevent homelessness. Engage consumers in treatment planning and ongoing service delivery through community outreach, relationship-building, and individualized support. Provide direct case management services in accordance with individualized care and treatment plans. Assist consumers with activities of daily living (ADLs) through coaching, modeling, and hands-on support. Support consumers in obtaining basic needs and benefits, including housing assistance, entitlements, transportation, and community resources. Monitor and assess consumers’ emotional well-being, psychiatric symptoms, behavioral changes, and medication compliance during all client interactions. Provide psychosocial education to consumers and family members regarding mental health symptom management, medication adherence, coping skills, and independent living skills. Respond to and support consumers during psychiatric or behavioral crises, including coordination with emergency screening services and psychiatric hospitals when necessary. Maintain collaborative working relationships with hospitals, community providers, and collateral contacts to support continuity of care and discharge planning. Protect consumer rights, dignity, confidentiality, and cultural values in all aspects of service delivery while maintaining accurate and timely documentation.
