The PRN Physical Therapist evaluates, plans, and provides patient-centered physical therapy services for individuals requiring rehabilitation. Responsibilities include completing evaluations, developing treatment plans, and documenting patient progress.
Candidates must have a current California Physical Therapist license and be a graduate of an accredited PT program. Previous experience in skilled nursing facilities or post-acute rehab is preferred but not required.
The PRN Physical Therapist evaluates, plans, and provides patient‑centered physical therapy services for individuals requiring rehabilitation in a post‑acute/long‑term care setting. You will collaborate closely with an interdisciplinary team to ensure the best rehabilitation outcomes for our residents. Responsibilities Complete PT evaluations and develop individualized treatment plans Provide effective, evidence‑based therapeutic interventions Document treatments, progress notes, and evaluations in accordance with facility and regulatory requirements Communicate patient progress with interdisciplinary team members Educate patients, families, and caregivers on therapy goals, safety, and home programs Ensure safe, ethical, and high‑quality care at all times Qualifications Current California Physical Therapist (PT) license Graduate of an accredited PT program (DPT or MSPT) Previous SNF or post‑acute rehab experience preferred (not required) Strong clinical reasoning and communication skills Ability to work independently and collaboratively Benefits (PRN) Competitive
