Provides social services and discharge management to patients and families to ensure a quality continuum of care. Additionally, the role involves precepting new employees and mentoring existing staff to enhance professional development.
Requires an LMSW license from the Texas State Board of Social Work Examiners. A minimum of 5 years of social work experience is preferred.
Provides social services and discharge management services to patients and families; these functions contribute to the quality of continuum of care. Also precepts new employees through the orientation process and mentors existing employees to enhance their professional development. Responsibilities include involvement with special projects upon assignment. Job Duties/Responsibilities: Completes pre-discharge evaluations from patient/family interviews and healthcare team collaborations. Based on evaluation, develops a viable plan of intervention for current needs and potential discharge needs with consideration of illness/injury, living situation, support systems and financial status. In addition, contributes to department’s ongoing professional enhancement thru role of preceptor and mentor and participation with assigned special projects.
