Assist with planning and conducting individual, small group, and large group activities Encourage resident participation in activities, hobbies, crafts, games, and reading Assist with resident outings and transportation coordination as needed Help develop and maintain monthly activity calendars and attendance records Provide in-room activities for residents who are bed-bound or isolated Assist with activity assessments, care plans, and quarterly progress notes Participate in resident care planning and discharge planning processes Maintain positive communication with residents, families, staff, and visitors Assist with Quality Assurance and activity program improvement initiatives Maintain the Activity Department in a clean, organized, and secure manner Promote a warm, engaging, and resident-centered environment Qualifications Education & Experience High school diploma or equivalent required One year of long-term care experience preferred Skills & Abilities Strong communication and interpersonal skills Creative, energetic, and compassionate approach to resident care Ability to follow written and verbal instructions Ability to work independently and as part of a team Physical Demands Frequent standing, walking, pushing, pulling, and reaching Ability to lift up to 50 lbs Occasional bending, kneeling, crouching, or crawling Ability to assist residents during activities and outings Work Environment Skilled nursing facility setting Low to moderate noise level Active, resident-focused environment We are an equal opportunity employer.
