The Care Coordinator screens patients for social drivers of health and connects them with internal and external resources. They are also responsible for data entry, documentation in Epic, and distributing material resources to patients.
A high school diploma is required, while an associate or bachelor's degree is preferred. Experience in community health or social services and bilingual fluency in English/Spanish are strongly desired.
Patient Support Responsibilities: Attend to patients through warm handoffs from the clinic. Screen clients for needs around social drivers of health and connect to internal and external resources. Provide linkage to services such as emergency financial assistance, transportation, food assistance, etc. Administrative Responsibilities: Complete data entry and documentation. Assist with distribution and tracking of SDOH resources, including bulk mailing of SDOH resources. This also includes Top Box Foods distribution, ordering Uber Health rides, and distributing other material resources. Use Epic to document work, respond to requests, process work queues, and communicate with clinical care teams. Performs other duties as assigned. QualificationsEducation Required: High School diploma or equivalent required.
Intermediate proficiency in Microsoft Office preferred Bilingual fluency (English/Spanish) strongly preferred, dependent on Erie site. Other bilingual fluency a plus.
