Manage health problems and coordinate comprehensive, compassionate care for patients and families facing serious illnesses. Formulate care plans and administer medications in collaboration with an interdisciplinary palliative care team across home, office, and SNF settings.
Requires a current California State NP or PA license and at least 6 months of clinical experience in Oncology, Geriatrics, Hospice, or Palliative Care. Specific certifications such as DEA registration and CAPA (for PAs) or a furnishing number (for NPs) are required.
Opportunity Information Palo Alto Foundation Medical Group is seeking a full time, certified Nurse Practitioner or Physician Assistant in Palliative Care. Palliative Care and Support Services (PCSS) is widely recognized nationally as an innovator in delivering integrated, multidisciplinary palliative care for an outpatient setting.
Managing health problems and coordinating health care for the patient in accordance with State Federal Rules and Regulations Provide comprehensive, coordinated, and compassionate care for patients and families facing serious illness Formulates a comprehensive care plan, patient education, and ongoing evaluation in collaboration with the palliative care interdisciplinary team for palliative care patients in the home, office and SNF settings Prescribes and administers medications/immunizations including controlled substances to the extent delegated and licensed, and in collaboration with palliative care and/or referring physician
