The Healthcare Navigator connects Veterans to VA health care benefits or community services through assessments and care coordination. They develop care plans with interdisciplinary teams and provide ongoing follow-up and health education to clients.
A Bachelor's degree in a related field is required, with a Master's level social work degree preferred. Candidates must have a valid Texas driver's license, auto insurance, and reliable transportation.
Recovery Resource Council is looking for a conscientious, detail-oriented, and compassionate individual who is capable of successfully connecting Veterans to VA health care benefits or community health services where Veterans are not eligible for VA care. The SSVF Healthcare Navigator (HCN) must be able to provide case management and care coordination and work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team.
To perform this job successfully, an individual must have a Bachelor’s degree in a related field. Master level social worker or equivalent education preferred. Experience working with people experiencing homelessness and/or veterans is preferred. Must have reliable transportation, a current Texas driver’s license and evidence of auto liability insurance.

This is a hybrid position, requiring both remote and in-person work, involving sitting, and standing for various lengths of time and frequently driving for long distances.
