The Community Health Navigator acts as a liaison between clinics and community resources to help patients access essential healthcare and social services. Responsibilities include assessing social determinants of health, providing health coaching, and coordinating referrals to external agencies.
An Associate's or Bachelor's degree in Public Health, Social Work, or a related field is required. Preferred qualifications include bilingual skills in English and Spanish and experience in community outreach or case management.
Represent AccessHealth at community meetings, events, and collaborative partnerships. Assess patients’ social determinants of health and identify needs. Provide health coaching and education to patients and community members. Assist individuals in accessing resources such as food, housing, health insurance, transportation, and healthcare services. Follow up with assigned patients to ensure successful access to care and services. Coordinate referrals and connect clients with appropriate external agencies. Participate in community outreach events, including health fairs and presentations. Accurately document all activities and services in patient records in a timely manner. Maintain ongoing communication with patients between visits. Serve as a liaison between the clinic and community organizations. Participate in ongoing training and professional development opportunities. Ensure compliance with organizational policies, procedures, and regulatory requirements. Perform additional administrative or related duties as assigned. Experience in community outreach, social services, resiliency, advocacy, or disaster recovery preferred. Ability to work both independently and collaboratively within a team. Experience with case management or client file management. Intermediate proficiency in Microsoft Office (Word, Excel, Outlook) and other systems. Professional and effective communication skills when interacting with clients and staff. Education Requirements Associate’s or Bachelor’s Degree required (preferred in Public Health, Social Work, or related field).
