About the Role

• As a Licensed Practical Nurse (LPN) at CarDon and Associates, you will play a crucial role in providing high-quality patient care across all shifts.
• Your primary responsibility will be to assist in the daily activities of residents, ensuring their comfort and well-being.
• You will collaborate with a multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
• Additionally, you will monitor patients' health status, administer medications, and provide education to both residents and their families.
• Ultimately, your contributions will enhance the quality of life for our residents and support their health goals.

• Job Responsibilities
• Administer medications and treatments as prescribed by the healthcare provider.
• Monitor and record vital signs and report any changes in resident conditions to the nursing staff.
• Assist residents with daily living activities, including bathing, dressing, and grooming.
• Maintain accurate and up-to-date resident records and documentation.
• Collaborate with healthcare team members to develop and implement care plans.

Minimum Qualifications

• Current and valid Licensed Practical Nurse (LPN) license in the state of Indiana.
• Basic Life Support (BLS) Certification.

Preferred Qualifications

• Experience in a long-term care or assisted living environment.
• Proficiency in electronic health record (EHR) systems.
