What you’ll do

As a Home Health Physical Therapist, you will

• Perform comprehensive patient evaluations and develop individualized treatment plans
• Deliver skilled physical therapy services to improve mobility, strength, balance, and overall function
• Help patients manage pain and prevent further physical limitations
• Monitor patient progress and adjust treatment plans to maximize outcomes
• Educate patients and families on exercises, safety, and disease management
• Collaborate closely with physicians, nurses, therapists, and interdisciplinary care teams
• Maintain accurate and timely documentation in accordance with company guidelines
• Uphold infection control and patient safety standards during every visit

• What we’re looking for
• Graduate of an accredited Physical Therapy program
• Current Physical Therapy license
• CPR Certification
• Valid driver’s license and auto insurance
• Home health experience preferred
• HomeCare HomeBase experience preferred
• Strong communication, clinical, and time management skills
• Passion for delivering patient-centered care in the home setting

• Ability to stand, walk, sit, kneel, squat, crawl, and climb for extended periods
• Ability to lift, carry, push, or pull up to 100 lbs.
