SUMMARY OF POSITION Provide nursing services within a 24/7 co-occurring residential treatment program serving individuals with serious mental illness and substance use disorders. The I-CRT program consists of two 8-bed INARR Level 4 recovery residences located on the same property, with one residence serving male clients and the other serving female clients. The program utilizes a person-centered, trauma-informed, and evidence-based approach to care. The RN works collaboratively with therapists, recovery coaches, behavioral health specialists, medical providers, and community partners to support psychiatric stabilization, medication management, physical health monitoring, crisis intervention, recovery engagement, and continuity of care. Responsibilities include nursing assessments, medication oversight, coordination with prescribers, health education, documentation, and monitoring client wellness and safety while maintaining compliance with Centerstone policies, DMHA standards, INARR Level 4 expectations, and best practices for co-occurring disorder treatment. ESSENTIAL DUTIES & RESPONSIBILITIES Provides direct patient care under the supervision of a Nurse Practitioner (NP), and/or Physician, evaluates outcomes, consults with other health team members as required and adjusts nursing care processes as indicated to ensure optimal patient care. Performs head-to-toe assessment, under the supervision of a NP, and/or Physician, on all patients as per policy. This includes neonate, pediatric, adolescent and geriatric patients and the general patient population. Adequately assess and reassess pain. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Supports and maintains a culture of safety and quality. Performs patient care responsibilities considering needs specific to the standard of care for patient’s age. Maintains current knowledge of medication and the correct administration based on age of the patient and his/her clinical condition. Medical and surgical asepsis is carried out during treatments and special procedures. Communicates appropriately and clearly to co-workers. Demonstrates the ability to assist NP/Physicians with procedures and perform services requiring technical and manual skills under the direction of an NP and/or Physician. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patient and family. Interacts professionally with patient and family and involves patient and family in patient education. Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; includes family in teaching, as appropriate. Demonstrates the ability to perform treatments and provide services to level of licensure. Meets current documentation standards and policies. Demonstrates knowledge of lab requirements, requisitions. Maintains established office policies and procedures, objectives, performance improvement program, safety, environmental and infection prevention and control standards. Maintains a safe, comfortable and therapeutic environment for patients and families in accordance with office standards. Answers telephone calls in a professional manner; refers patient problems to NP; schedules patient appointments. Ensures an adequate stock of supplies, including vaccines. Assists in cost containment through appropriate ordering and conserving of supplies and equipment. Maintains a good working relationship with all co-workers. Manages and operates equipment safely and correctly. Performs preventive maintenance. Demonstrates the ability to be flexible, organized and function under stressful situations. Response is appropriate to the patient in emergency or physically distressful situations. KNOWLEDGE, SKILLS & ABILITIES Effectively communicate via written, verbal, in person and virtual methods.
