Yoakum Community Hospital is seeking a dedicated and highly skilled Social Worker to join our interdisciplinary team. In this role, you will be the cornerstone of patient advocacy and mental health support, providing high-quality care through expert assessment, psychodiagnosis, and treatment planning.
Conduct psychosocial evaluations, develop individualized treatment plans, and provide evidence-based therapy. Manage the discharge planning process from admission to completion, utilizing a deep knowledge of community resources. Maintain timely and accurate clinical documentation, progress notes, and assessments that align with state/federal regulations. Familiarity with trauma-informed approaches and clinically supported treatment methods. Current Texas LCSW License required. Experience in a hospital or clinical office setting is highly preferred.
