Lafayette Franciscan Healthy Living Clinic 1116 N 16th St Lafayette, Indiana 47904 The PACE Personal Care Attendant provides psychological, physical and emotional support and answers questions as appropriate through exemplary teamwork serving as many seniors as possible with the best quality-of-life in their communities.

WHAT YOU CAN EXPECT Practice Hours: Monday – Friday, 8:00 a.m. – 5:00 p.m.
No Weekends, Evenings, or Holidays Respond to participant needs and direct the inquiry to the appropriate party.
Maintain inventory supplies, sets-up of the care environment, and physically assist PACE participants.
Deliver care in multiple settings, including in the home, the day center, the clinic or in transportation.
