Provide support to address social drivers of health.\nQualifications\nREQUIRED\nGraduate of an accredited Registered Nursing School (RN) \nCurrent License in the State of Georgia as a Registered Nurse or Nursing Compact (NLC/eNLC) or Multistate License \nTwo (2) years previous clinical experience.\nPREFERRED\nBSN\nThree (3) years of experience in case management, social services or related field.\nCertified Case Manager.\nAbility to interact with patients, families, physicians and hospital staff.\nExcellent oral and written communication skills.\nAbility to type accurately at an approximate rate of 30 wpm.\nWork Hours:\n2-1030p \nWeekend Requirements:\nYes- Rotating Basis \nOn-Call Requirements:\nNo
