• Duties to include:
• Conduct comprehensive in-home physical therapy evaluations
• Develop and implement individualized treatment plans focused on mobility, strength, balance, and safety
• Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management
• Educate patients and caregivers on home exercise programs, safety strategies, and equipment use
• Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care
• Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards
• Recommend adaptive equipment and home modifications when appropriate
• Monitor patient progress and adjust treatment plans as needed
• Communicate changes in condition or concerns to the interdisciplinary care team

We care for each person with empathy, understanding, and love. Qualifications Licensed to practice as a Physical Therapist in the state of Iowa. Minimum of two years of experience in physical therapy; community/home health experience preferred. Strong communication, organizational, and problem-solving skills. Valid driver’s license with reliable transportation.
