Coordinate and deliver individualized end-of-life care for hospice patients and their families across various settings. Collaborate with an interdisciplinary team to perform assessments and adjust care plans for optimal outcomes.
Requires an active Registered Nurse (RN) license in Massachusetts and current CPR/BLS certification. Candidates must have a reliable vehicle for travel and experience with Point of Care EMR documentation.
Active Registered Nurse (RN) license in the Commonwealth of Massachusetts (in good standing) Prior hospice experience preferred Ability and willingness to travel within the service area using a reliable, insured, and registered vehicle Experience with Point of Care EMR documentation Current CPR / Basic Life Support (BLS) certification Strong clinical, organizational, and communication skills Ability to work independently while collaborating effectively with a care team
