• Ennoble Care is seeking a full-time Care Coordinator to join our team in an office-based setting in the College Park area of Atlanta, Georgia.
• This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice-style model of care.
• While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.

• KEY RESPONSIBILITIES Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner Assist with care transitions, referrals, and coordination with external partners, as needed Collaborate closely with fellow Care Coordinators and internal care team members within an office-based environment Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision-making and efficient clinical operations Contribute positively to team collaboration, workflow efficiency, and day-to-day office operations
