Provide skilled nursing services to psychiatric and opioid-dependent patients, including assessment, medication administration, and crisis intervention. Coordinate care with multi-disciplinary teams and provide essential education to patients and their families.
Requires a Bachelor's degree in nursing, a Colorado RN license, and five years of nursing experience, preferably in psychiatric settings. Must have at least one year of experience in the substance abuse field and obtain BLS certification within 90 days.
The role of the Nurse, RN provides skilled nursing services to psychiatric and/or opioid dependent patients and their significant support systems.
Our ideal candidate for our Nurse, RN position will have bachelor's degree in nursing and licensed as a Registered Nurse in the State of Colorado. They will have five (5) years nursing experience, preferably in a psychiatric/mental health setting, knowledge of psychiatric medication and one (1) year experience in substance abuse field. They will also have experience working with a multi-disciplinary team and working with mental health conditions.
Assess needs of client, develop solutions and ensure implementation of solutions/interventions and programs to meet those identified needs. Observes, identifies, monitors, reports and records the condition of the client including signs and symptoms which may be indicative of change in the consumer’s condition to appropriate members of the treatment team (includes treating psychiatric provider and clinical staff). Follows State and Federal guidelines in providing care to opioid dependent clients in collaboration with licensed prescribers and pharmacies. Provides care to patients in a manner that recognizes the human potential, is empathic, non-judgmental, and demonstrates unconditional positive regard while promoting safety, comfort and the therapeutic environment. Provides assistance in crisis intervention, as needed. If assesses situation to be emergent; calls crisis team, notifies provider, and may facilitate transfer to an emergency department if appropriate. Acts as a liaison to outside agencies and providers for clients to ensure continuity of care. In this the capacity advocates for client’s needs, ensures care provided and ensures necessary follow-up. Provides patient and family education, either in groups or individually, including health education, medication education and/or education related to substance use disorder, psychiatric illness, and techniques/skills for physical care. Provides case management and other clinical interventions as appropriate. Participates as needed in care coordination; collaborative development and implementation of client’s treatment plan with members of the treatment team. Maintains confidentiality of patient records. Documents intervention and program activity according to Axis Health System’s policies and procedures. Attends and participates in nurse meetings, in-services education and training opportunities, and special events as indicated. Medications Administer according to Axis policy. Document accurately immediately following med administration. Store according to AHS policy. Reports medication errors according to Axis policy. Follow proper med disposal procedures per Axis policy. Participate in C 2-5 medication counts according to Axis policy. Monitor/assist with medication refills according to Psychiatric Services protocol/policy. Program Assignments Performs chart checks as required by Program according to Axis protocol/policy. Additional eligibility requirements: Annual Flu immunization, Annual TB screening, de-escalation training within 90 days of hire, BLS certification required within 90 days of hire.
