The Care Coordinator provides intensive community-based support to children and families who have experienced trauma, helping them navigate behavioral healthcare transitions. Responsibilities include case management, coordinating with multidisciplinary teams, and maintaining relationships with community partners.
Requires a Bachelor's degree in psychology, social work, or a related field and 1-3 years of relevant experience in behavioral health. A valid Colorado driver's license is mandatory for travel to client homes and community sites.
The salary range for this position is SummitStone’s good faith belief and reasonable estimate of the range of possible compensation for this position at the time of the posting. The Encompass Care Coordinator works on a small, multidisciplinary team that provides intensive community-based services to children and families who have experienced trauma. The Care Coordinator provides direct support to families in navigating transitions in care, including through referral, intake, and stepdown into less intensive treatment. The Care Coordinator also provides case management services as needed and supports program tracking and data collection efforts. The ideal candidate for this position will be motivated and take initiative, have strong communication skills while interacting with clients, staff, and community partners, and be detail-oriented and willing to follow and develop essential workflows.
• Valid Colorado driver’s license or the ability to obtain one within 30 days of hire required; must maintain a clean driving record. This role requires travel to client homes and community partner sites.
