The Nurse Practitioner will provide express care services, including performing physical exams, diagnosing conditions, and developing therapeutic plans. Responsibilities also include suturing wounds, ordering diagnostic tests, and coordinating continuity of care for patients.
Candidates must be a graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant program and hold a valid Vermont license. A minimum of 3-5 years of experience in Express or Urgent Care practice is required.

Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.
