Collaborating closely with medical, nursing, rehabilitation, and social services teams, you’ll address chronic conditions, functional challenges, and age-related nutrition needs—helping older adults thrive while maintaining dignity and independence.
Key Responsibilities Clinical Nutrition Care Conduct comprehensive nutrition assessments upon enrollment, semi and annually, and with any significant change in participant condition.
Develop, implement, and monitor individualized nutrition care plans based on medical history, functional status, cultural preferences, and psychosocial needs.
Provide medical nutrition therapy (MNT) for chronic diseases such as diabetes, cardiovascular disease, renal disease, malnutrition, and dysphagia.
Minimum of 1–2 years of clinical nutrition experience (geriatrics, long‑term care, or community health preferred).
