The Physical Therapist is responsible for evaluating patient clinical needs, developing and implementing plans of care to meet national benchmarks, and ensuring appropriate utilization of services based on HHRG values. This role also involves instructing and supervising Home Health Aides, coordinating discharges, and maintaining timely, clear, and concise documentation of skilled care.
Candidates must possess a Bachelor of Science or Master's Degree in Physical Therapy from an accredited institution, hold a current PT license in Massachusetts, and have one year of clinical experience along with CPR certification. Computer skills are preferred for this role.
Bachelor of Science Degree or Master's Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy Association. Current PT license in Mass. with one (1) year clinical experience and CPR certification required. Computer skills preferred.
