The Physical Therapist is responsible for maintaining national standards for HHRG values, establishing appropriate visit frequency, and developing, implementing, and revising patient plans of care based on clinical needs. This role also involves participating in interdisciplinary conferences, supervising Home Health Aides, notifying providers of significant changes, and ensuring timely and accurate documentation of skilled care.
Candidates must possess a Bachelor of Science or Master's Degree in Physical Therapy from an accredited institution and hold a current Physical Therapist license in Massachusetts, along with one year of clinical experience and CPR certification. Computer skills are preferred for this role.
Bachelor of Science Degree or Master's Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy Association. Current PT license in Mass. with one (1) year clinical experience and CPR certification required. Computer skills preferred.
