The Ambulatory Clinical Pharmacist optimizes therapeutic outcomes for patients with chronic conditions through evidence-based treatment and medication adherence strategies. They collaborate with interdisciplinary teams to resolve medication-related problems and provide individualized patient counseling.
Requires a Doctor of Pharmacy (PharmD) degree and an active Texas license. A minimum of 3 years of experience as a clinical pharmacist is required, with PGY1/PGY2 residency preferred.
