The Patient Care Coordinator facilitates efficient back-office workflows and coordinates care between providers, patients, and community partners. Key duties include scheduling, case management for high-risk patients, and assisting medical providers in the clinic.
Candidates must have a high school diploma, a current Oregon Driver's License, and CPR certification. CCMA or LPN certifications and high-level clinical work experience are preferred.
