The role involves coordinating medical, social, housing, and educational services for mentally ill children, adolescents, and adults. Responsibilities include implementing case management plans, maintaining Medi-Cal documentation, and providing direct services to clients.
A bachelor's degree in social work, counseling, or a related field is required along with one year of relevant experience. Proficiency in Spanish is preferred, and candidates must possess a valid driver's license and pass background and drug screenings.

The Personal Service Coordinator/Case Manager III reports to the PSC Supervisor and Assistant Program Director and is responsible for providing services which will assist individuals in gaining access to Medi-Cal eligibility, medical, social, housing, educational and other services.
