Lead Care Planning Teams to coordinate services for youth with serious emotional disturbances and their families. Serve as a liaison between agencies and provide trauma-informed wraparound services in home and community settings.
A Bachelor's degree in Social Work, Psychology, or a related field is required, with a Master's degree preferred. Candidates must be at least 21 years old, possess a valid driver's license, and be available for flexible evening hours.

Family-Based Intensive Treatment (FIT) is a new service within JRI's Community Service Agencies that provides a combination of medically necessary intensive family therapy, robust care coordination, and Family Partner engagement for eligible youth with serious emotional disturbance (SED) and their families. You will focus on improving the youth’s mental health concerns, while strengthening the family structure and support. Our staff provide the FIT service in home and community settings and facilitate a consistent, strength-based therapeutic relationship with the families we serve.
The pay range for this position is $22.84 to $26.00 per hour. The listed range reflects what we reasonably and in good faith expect to pay for this role at the time of posting the position. The actual compensation an employee is offered may vary based on several factors such as experience, education, licensure, certifications and qualifications. This range may be modified in the future.
This position covers the Merrimack Valley region and requires availability to work from the afternoons into the evenings. While weekend hours are available, they are not mandatory. The scheduling for this role is flexible and will be adjusted based on your availability as well as that of the client or family.
