Provide comprehensive medical social work support, focusing on patient-centered care and optimal discharge planning. Collaborate with multidisciplinary teams to conduct biopsychosocial assessments and coordinate services with community partners.
Requires a BSW or MSW and valid licensure as a CSW, APSW, or LCSW from the Wisconsin Department of Safety and Professional Services. Previous experience in a healthcare setting is preferred.
As a social worker you will provide comprehensive support across various medical social work areas, including hospital inpatient and transitional care. This position serves as a critical resource within multidisciplinary teams, focusing on patient-centered care and collaboration to ensure optimal discharge planning and coordination of services.
