Coordinate and direct the delivery of care for a caseload of hospice patients in a home setting. Develop and revise individualized plans of care in collaboration with physicians and interdisciplinary teams to achieve patient goals.
Requires a nursing diploma or degree, a valid PA/NJ driver's license, and an active RN license in Pennsylvania. Candidates must have at least one year of recent direct healthcare experience and BLS certification.

Associate’s Degree Nursing or Bachelor’s Degree Nursing 1 year experience as a home health or hospice Registered Nurse Knowledge and ability to navigate multiple computer applications and EMR platforms. AMB-BC- Ambulatory Care Nursing-Board Certified - American Nurses Credentialing Center within 3 Years or CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years Physical Demands Lift and carry 40-50 lbs.
Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
