Perform skilled nursing visits, including admissions and discharges, for hospice clients of all ages in their residences. Coordinate care with the interdisciplinary team, patients, families, and referring agencies.
Requires a nursing degree and a current Texas Registered Nurse license. At least one year of clinical care experience, specifically in acute settings involving physical assessments, is preferred.
SUMMARY Primary functions are to perform skilled nursing visits including admissions and administer nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assume the responsibility for coordination of care. DUTIES AND RESPONSIBILITIES Skilled visits as assigned. Admit, Recert, SCIC, Post-hospital and Discharge visits as assigned Supervisory hospice visits to patients Preparing home health aide care plans Check charts for completeness and accuracy Attend staff meetings On-call rotation Other assigned duties. JOB QUALIFICATIONS Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Licensure: Current Texas State license as a Registered Nurse, current Texas Driver’s License Experience: One year experience as nurse in a clinical care setting.
Nursing skills as defined as generally accepted standards of practice. Knowledge of State and Federal regulations. Good interpersonal skills.
Reliable transportation. Valid and current auto liability insurance. EXPERIENCE REQUIRED One (1) year of experience in an acute setting that requires heart, lung and complete physical assessment; or demonstrated ability of education in the same areas is preferred. ENVIRONMENTAL AND WORK CONDITIONS Works in patient’s home in various conditions and locations; possible exposure to blood and bodily fluids and infectious diseases; Proof of current CPR, and Hepatitis consent/declination; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
