Deliver skilled physical therapy visits in patients' homes following a supervising PT's plan of care. Focus on improving functional mobility, managing patient outcomes, and maintaining high documentation standards in Homecare Homebase.
Must possess an active Michigan PTA license, a valid driver's license, and reliable transportation. Home health experience is strongly preferred along with competency in core PTA scope and safety-first mobility work.
Active Michigan PTA license (in good standing) Reliable transportation, valid driver's license, auto insurance Home health experience strongly preferred Able to carry/transport basic therapy equipment as needed Competency in core home health PTA scope Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations Strengthening, functional mobility, and home safety assessment/modification education Patient/caregiver coaching with practical home programs Our guarantee Full-time target: ~30 productivity points/week; clinicians earn more for exceeding it. Guaranteed floor: ~24 points/week for the first 90 days - if your caseload hasn't filled, you're still paid the floor. The rest of the package $1k sign-on bonus paid after first 90 days $1k bonus for referrals (we’re growing)! Benefits for full-time (medical/dental/vision, PTO)
