Develop, implement, and adjust care plans based on patient needs, ensuring all aspects of care meet high clinical standards. Communicate and coordinate with families, therapists, social workers, and HHA staff to ensure a well-informed care team. Advocate for patient needs and provide ongoing education to families and caregivers, empowering them to support the patient’s care effectively. Prior experience in home health or healthcare admission is preferred. Strong assessment, organizational, and communication skills. Ability to work collaboratively with a diverse healthcare team and demonstrate empathy toward clients and their families. Valid driver’s license and reliable transportation required for travel to client homes.
