Requires an Associate's Degree from an APTA accredited Physical Therapy Assistant program and at least 2 years of PTA experience. Must hold a South Carolina PTA license and obtain a SC Driver's license within 90 days of hire.
Job Summary Provides physical therapy treatment under the direction of a licensed Physical Therapist according to the principles and practices of Physical Therapy.
Collaborates with team members in the implementation of the patient's individualized plan of care.
Provides patient/family education, completes documentation of patient treatments, charges and other information as needed.
Added responsibilities for program and staff development is required.
Advanced level of clinical and interpersonal skills are required.
Accountabilities Implements and manages a specific and comprehensive treatment plan and participates in patient discharge planning: 50% - Administers patient treatments within the plan of care (POC) outlined by a licensed physical therapist and progresses patients as indicated - Demonstrates basic knowledge of diagnoses/conditions seen in physical therapy - Interacts with others involved in the patient's care to incorporate an interdisciplinary treatment approach for coordinated quality care - Demonstrates awareness and use of proper safety measures regarding the patients, others in the home and self - Assesses patient response to treatment and modifies goals and treatments as indicated - Provides information regarding follow-up services which may include a referral to other community resources Actively participates in patient and family education process.
15% - Supports and provides educational material to patient/families for support groups as well as community resources - Provides education and training to patients, families, and caregivers regarding diagnoses, progress, and long-term functional outcomes - Teaches, counsels, and communicates with the patient, family or caregiver educational information related to the patient's diagnosis/condition Complies with departmental policies for documentation.
15% - Documentation is in compliance with chart review audit components - Documents all aspects of assessment, planning, treatment and discharge in the computer in a timely manner - Follows POC as defined by the primary therapist and documents the patient's progress toward goals - Daily treatment records are noted in the computer on a daily basis and are clear and professional in terminology. - Appropriate abbreviations and medical terminology is utilized - Obtains re-evaluations as required by S.
