The LPN provides professional nursing care in a Transitional Care Unit, including implementing practitioner orders and managing medications. They are responsible for monitoring resident health status, documenting changes, and supervising unlicensed staff.
Candidates must hold a valid MN LPN licensure and AHA BLS or Red Cross CPR certification. Strong communication skills and the ability to work independently are required.
