The LVN assists in identifying patient needs and provides physician-ordered services under the direction of an RN. Responsibilities include patient and caregiver education, clinical documentation, and ensuring continuity of care in home settings.
Candidates must be a graduate of an accredited vocational/practical nursing school with an active state license. At least one year of clinical care experience is required, with home health and Homecare Homebase experience preferred.

Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation. You will be expected to be present within our partnering communities and our clients independent residences. Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner. Monitors assigned cases to ensure compliance with requirements of third-party payor. Participates in the Agency’s QAPI program. Participates in the Agency sponsored in-service trainings. Demonstrates commitment, professional growth, and competency. Promotes the Agency philosophy and administrative policies to ensure quality of care. Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care. Provides patient and caregiver education. Assures the continuity of care through delivery of quality patient care. Provides care using infection control measures that protect both staff and patient.
Education: Graduate of an accredited school of vocational/practical nursing.
