Job function
Responsible and accountable for performing direct and indirect patient care according to standards set for quality and compassionate patient care and those duties approved by the Louisiana State Board of Practical Nursing Examiners.
Responsible for charting all pertinent information concisely on patient records in a timely and legible manner.
Appropriately and professionally communicates between staff members, management, and physicians.
The LPN functions as an integral part of the nursing team.
All tasks must be performed in compliance with established policies and procedures of Allen Parish Community Healthcare and supervised by the Registered Nurse and/or the Unit Nurse Manager, and/or the CNO.
Additional responsibilities
Performs assigned nursing procedures for the comfort and well- being of the patients and any additional duties of functions for which a LPN is allowed by Louisiana state law and the organization’s policy and procedures and/or as delegated by the RN.
Assists RN with admission, education, discharge planning, and discharge of patient including any necessary documentation.
Monitors patient’s condition by making frequent rounds on assigned patients, gathering data necessary to administer care, administers appropriate care (including isolation-type environments) and properly and accurately documents such findings.
Properly and accurately administers medication(s) as prescribed by Physician/NP after verifying with Physician/NP orders, properly identifying the patient utilizing two identifiers (not the patient’s room number), and provides basic nursing care needs to patients (hygiene, comfort, nutrition, etc.)
Properly collects, labels, stores, and/or delivers specimens such as urine, sputum, etc. utilizing proper universal and/or isolation precautions according to organization’s policy and procedures.
Performs the following procedures:
Sterile dressing changes
NG tube irrigation
Catheterizations
Enemas
Measurements of drainage
Suctioning nasopharyngeal
Suctioning tracheotomy
Tracheotomy care
NG tube feeding: continuous, intermittent
Compresses
Perineal care
Wound care
Other procedures directed by the RN
Monitors drip rate IV’s, infusions, and adds IV solutions to previously started IV’s.
Proficient in Med Dispense usage.
Monitors, calculates, and records accurately patient intake and output and records such finding in the patient’s chart.
Assists with:
Transporting patients
Ambulating patients
Delivering diet trays
Feeding patients
Routing AM and PM care
Oral hygiene
Other duties including, but not limited to, politely and promptly answering and appropriately responding to patient call bell.
Assists RN, Physician, and/or NP with procedures when needed and makes rounds with Physician/NP as needed.
Attends and participates in required monthly staff meetings (according to employment status), in-services, select committees, and annual hospital orientation.
Maintains a working knowledge of and adheres to all nursing and hospital policies and procedures.
Must have the ablility to identify and address the special needs of all patients in pertinent areas including: Neonates/Infants-birth to 12months, Toddler-1-3 years, Pre-Schooler- 4-5 years, School Age/Adolescent- 6-17 years, Adults- 18-64 years and Geriatrics- 65 years and above.
The LPN will promote teamwork by interacting and communicating with fellow employees in a way that promotes a harmonious and cooperative working environment.
Accurately receives, verbally repeats, and documents Physician/NP order(s); reporting such to the RN properly signing off orders to include name, date, and time of executing the order(s).
At times of need, may be asked to work varying shifts.

Current licensure as a LPN in the State of Louisiana or compacted state.
BLS/CPR certification is required
CPI/MOAB certification is required

PALS certification is preferred
Education
• 1 year experience as LPN in the Acute care setting preferred.
