The Clinical Pharmacist manages medication therapy and ensures patient safety through accurate dispensing, clinical reviews, and collaboration with healthcare providers. They are responsible for patient counseling, monitoring therapeutic outcomes, and implementing evidence-based clinical protocols.
Candidates must hold a Doctor of Pharmacy (PharmD) degree and have at least two years of clinical experience or a PGY1 residency. Proficiency in EHR systems, Microsoft Office, and strong clinical judgment are required.
Description The Clinical Pharmacist ensures the safe and effective use of medications by providing comprehensive clinical pharmacy services, including medication management, patient education, and collaboration with healthcare professionals to enhance patient care. This role prioritizes medication safety, evidence-based practice, and quality improvement while supporting optimal patient outcomes.
LEVEL OF AUTHORITY & RESTRICTIONS Exercises independent clinical judgment in medication management while collaborating with healthcare teams. Functions within established clinical protocols, guidelines, and regulatory standards; coordinating care with clinic providers and escalating complex or high-risk cases as appropriate.
WORKING CONDITIONS & ENVIRONMENT May require working occasional nights and/or weekends. Moderate noise level with frequent interruptions and distractions. Must be willing and able to travel both locally and within the CTCLUSI service delivery area and work at locations other than Three Rivers Health Center.
Must be 18 years of age or older. A Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy. Minimum of two (2) years of clinical pharmacy experience or completion of a postgraduate pharmacy residency (PGY1). Experience and proficiency in the use of Microsoft products (Excel, Outlook, PowerPoint, Word, etc). Demonstrates comprehensive knowledge of pharmacotherapy, medication safety principles, and applicable regulatory requirements. Applies sound clinical judgment to ensure patient safety, including identifying and managing medication-related risks, contraindications, and potential drug interactions. Utilizes strong clinical judgment and analytical skills to assess patient needs and optimize therapeutic outcomes. Excellent verbal and written communication skills to effectively counsel patients and collaborate with healthcare professionals. Proficient in using electron health records (HER) and pharmacy information systems for documentation and medication management. Ability to prioritize tasks, manage time effectively, and adapt to changing clinical demands in a fast-paced environment. Ability to communicate clearly and effectively in English, verbally, in writing or by other acceptable means. This position is considered a covered role per the CTCLUSI Background Investigations Policy. A state criminal background check and fingerprint-based background check will be required as a condition of employment. This position is designated as safety-sensitive and is subject to pre-employment and other authorized drug and alcohol testing in accordance with company policy. Please note that the use of marijuana is prohibited for employees in this position, regardless of state legalization status.
