Provide intensive home and community-based assessment, planning, and treatment for families at risk of child removal. Coordinate with collateral resources and manage clinical decisions in partnership with a Case Manager.
Requires a Master's degree in Social Work, Mental Health Counseling, or a related field with at least one year of post-graduate experience. A valid driver's license is mandatory, and professional licensure (LMSW, LCSW, LMHC, or LMFT) is preferred.
Provide home and community-based assessment, planning and treatment with individuals, groups and families with those enrolled in our Family Preservation services (the level of intensity / number of direct services hours per family is dependent upon the services in which one is employed) Complete necessary all records and client information following agency, county, and state policy and funding requirements Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment Responsible for the clinical decisions when assigned and paired to work as a team with a Bachelors Clinical Case Manager. Ensure provisions of regular treatment plan meetings and services plan reviews.
