The Care Team Hospice is seeking a part-time CNA to provide personal care and comfort-focused support to hospice patients in their homes and in facility settings, working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey. • Standard operating hours are
• Monday through
• Friday, 8:00
• AM to 4:30 PM
• This role will cover Tawas, MI and territories within an approximate one-hour radius of the market.

• Provide personal care, hygiene, and comfort measures
• Observe and report changes in patient condition
• Support patients and families with compassion and respect
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase) QualificationsQualifications
• Active CNA certification
• Hospice aide experience preferred
• Experience using HCHB (Homecare Homebase) preferred Field Role Requirements
• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
