We are seeking a dedicated and compassionate Physical Therapist (PT) to join our PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you will evaluate participants, develop and implement personalized treatment plans, and support individuals in achieving optimal function and independence. The Physical Therapist works both in the PACE Center and in participants’ homes, collaborating closely with an interdisciplinary team (IDT) to ensure coordinated, high-quality care. This role involves hands-on patient care, education of participants and caregivers, and active participation in care planning and quality improvement initiatives. Key Responsibilities Clinical Care & Patient Management Perform comprehensive initial assessments and ongoing reassessments per CMS and PACE guidelines Develop and implement individualized treatment plans in collaboration with the IDT Administer evidence-based physical therapy treatments and monitor participant progress Adjust treatment plans based on functional outcomes and participant response Educate participants, families, and caregivers on exercises, safety techniques, and use of equipment Evaluate and recommend durable medical equipment (DME), home modifications, and adaptive devices Conduct home safety assessments and coordinate modifications to support independence Interdisciplinary Team Collaboration Actively participate in IDT meetings and care planning discussions Communicate participant status, changes, and treatment outcomes to the care team Collaborate with physicians, nurses, and other disciplines to ensure coordinated care Maintain open communication with leadership regarding concerns or needs
Education & Licensing Master’s Degree in Physical Therapy (required) (Bachelor’s degree considered if grandfathered) Current Physical Therapist (PT) license BLS/CPR certification (or ability to obtain within 60 days) Experience 2–3 years of Physical Therapy experience required Experience with geriatric or frail populations (preferred) Experience in interdisciplinary team settings (preferred) Familiarity with Electronic Medical Records (EMR) Key Competencies Strong clinical assessment and treatment planning skills Knowledge of geriatric care and rehabilitation strategies Experience with therapeutic modalities (exercise, mobilization, heat/ice therapy) Ability to evaluate functional mobility and ADL performance Excellent communication and interpersonal skills Strong documentation and organizational abilities Ability to work independently and collaborate in a team-based environment Knowledge of home safety risks and adaptive solutions Calm and effective response in emergency situations Work Environment Care delivered in both PACE Center and participants’ homes Collaborative, interdisciplinary team setting Fast-paced environment requiring multitasking and flexibility Why Join Us?
