The Social Worker Assistant provides administrative support to social work staff, focusing on the fulfillment of goods and services for participants. Key duties include assisting with Medicaid recertification, researching community resources, and updating participant demographic information.
Requires an Associate's degree in healthcare or social services (or equivalent training) and two years of experience in community-based social services or healthcare. Candidates must have a valid driver's license, personal transportation, and at least one year of experience working with older adults or people with disabilities.
The SWA may be assigned tasks related to locating and validating participant location, telephone number, address, emergency contacts and other demographic information necessary for ongoing engagement with the participant. Additionally, at the direction of the Social Work Supervisor, the SWA may be responsible for updating key demographic information in InnovAge systems as defined by policy or regulatory requirements. Travel Requirements Travel Travel may be required within the Center service area.

CPR/BLS and First Aid required in California only. Work Experience and Qualifications Two years’ experience in community-based social services organization or health care setting required. Experience in connecting clients with home and community services, applying for/recertifying Medicaid/Medical eligibility, performing client fulfillment activities, or other similar experience required. Requires personal transportation, current state issued driver’s license, good driving record and auto insurance as required by law. A minimum of one year experience working with older adults and people with disabilities PREFERRED Work Experience and Qualifications Three years or more of experience working in human, healthcare, or community-based services for older adults and people with disabilities.
