Provide direct medical care and health education to adults with intellectual and developmental disabilities in a residential setting. Coordinate care with community medical providers and facilitate health and wellness groups for individuals.
Requires an Associate degree in Nursing and a valid Massachusetts driver's license. Candidates must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN), with experience in developmental disabilities and MAP certification preferred.
Provide direct medical care as ordered by a physician Educate and consult with individuals and staff on medical conditions, medications, and general health topics Deliver individualized training and facilitate health and wellness groups Assist individuals in accessing and utilizing community health resources Guide individuals in understanding their medical conditions including symptoms, complications, and treatment options Review medical documentation and provide care coordination and consultation as needed Coordinate and follow up on medical care and attend appointments as assigned Provide side-by-side interventions focusing on health education and skills training Support the team in assessing symptoms and treatment effectiveness and recommend appropriate actions Build relationships with community medical providers and VNA services to enhance care coordination and act as a liaison between individuals, providers, and the treatment team
Associates degree in Nursing or equivalent Experience working with adults with intellectual or developmental disabilities injuries Valid Massachusetts drivers license Licensed Registered Nurse (RN) or Licensed Practical Nurse MAP certification preferred Ability to travel between Lexington, Bedford, Woburn and Arlington(mileage reimbursed) Schedule: 8am-4pm or 9am-5pm (40 hours) Hourly Rate $30.13—$30.13 USD At Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
