The Case Manager is responsible for completing assessments and Individual Service Plans while documenting progress notes according to DDS/MRC standards. They coordinate service delivery, advocate for consumers with outside agencies, and maintain professional relationships with families and providers.
Candidates must have a Bachelor's degree or equivalent experience and a history of working with adults with intellectual disabilities or brain injuries. A valid driver's license and reliable vehicle are required to service the designated community areas.
Assist in completing assessments and Individual Service Plans(ISP)/Plans of Care(POC) for assigned cases in a timely manner and as indicated by DDS and MRC policies and standards. Ensure individuals are involved in identifying and developing their own goals. Document daily progress notes for all individuals in accordance with Eliot Policy and DDS/MRC documentation and billing requirements. Develop and maintain a positive and professional working relationship with individuals, families and providers. This includes maintaining a set schedule with individuals, timely communication and follow up on issues and requests and being on time for appointments, meetings and activities. Coordinate service delivery for cases assigned i.e., case conferences, Section 8 Housing, SSI and advocate for the consumer with outside agencies and providers. Provide service linkages to other providers as necessary. Identify and schedule appointments, for/with individuals, as needed or required.
Bachelor’s degree or equivalent experience Experience working with intellectually disabled or brain injured adults. Ability to manage a client caseload in an outreach setting. Valid driver’s license and a reliable vehicle. Ability to work a community based position that services the Lawrence, Lowell, Burlington and Arlington areas. Main office located in Lexington MA.
