Perform psychosocial assessments and provide skilled individual, group, and family therapy for patients with co-occurring psychiatric and medical diagnoses. Coordinate team meetings, manage documentation compliance, and establish community linkages to ensure continuity of care and timely discharge.
Requires a Master's degree in social work and valid licensure as a LCSW or LICSW in Massachusetts. Preferred qualifications include LICSW licensure, proficiency in EMR systems, and multilingual fluency.
The Department of Mental Health (DMH) Tewksbury Hospital Hathorne Units an experienced, dynamic and compassionate candidate to serve as a Clinical Social Worker (AB). The selected candidate will enhance the level of service provided to clients on the Medically Enhanced Unit (MEU) while supporting the DMH initiative for recovery based practices and Person Centered Planning. The MEU serves patients with co-occurring psychiatric and medical diagnoses such as end stage renal disease, cancer, eating disorders, failure to thrive or in need of short term rehabilitation etc. The MEU also supports patients and families through end-of-life care and navigating advanced care directives.

Joint Commission accredited LTAC hospital that provides high quality, comprehensive treatment, care, and comfort to adults with chronic medical and mental illness. A full range of services are provided including 24/7 Nursing, Nurse Practitioners, Medicine as well as psychiatric and psychological services, a 6- bed dialysis unit, clinical laboratory, radiology, respiratory care, nutrition, speech/language, pathology, occupational therapy, physical therapy, therapeutic recreation and arts, and pharmacy.
